Yes, telehealth appointments are available. Please call our office to schedule an appointment.
If the patient has had a well check-up within the past year, you don’t need an appointment for forms (e.g. physical forms, sports forms, school forms, medication forms, WIC forms, excuse notes). If the patient has not had a well check-up within the past year, you must first schedule an appointment for a doctor to see the patient.
Yes, we welcome soon-to-be parents to schedule a free “Meet the Physician” visit at our office. We discuss what to expect in those first exciting months after birth, basic newborn care, how our office works, and give you a familiar face to help greet your new arrival.
For new patients, please complete the new patient packet before your first visit. If you have immunization cards or any past medical records, please bring them. For returning patients, please bring any uncompleted forms.
Our office phone number can be called 24 hours a day, 7 days a week. The office staff answers calls during business hours, and we do not close the phone lines for lunch. If calls come in after hours, the caller has the option to leave a voicemail, or get transferred directly to Dr. Lee’s personal phone.
To ensure the fair, timely, and quality care for our patients, our clinic strictly enforces a “No Show / Cancellation Policy". If you have any questions please let us know.
  • 24-Hour Notice Required: Our practice requires that for non-emergency situations, you must notify us at least one business day (24 hours) in advance if you need to reschedule or cancel an appointment. Failure to do so within this time frame will be recorded as a "No Show" appointment.
  • Cancellation Method: Cancellations cannot be made via voicemail or email. Please contact our office directly to cancel or reschedule your appointment.
  • Same-Day Bookings: If you book an appointment within a 24-hour period and subsequently cancel, the no-show policy applies immediately.
  • Late Arrivals: Arriving later than halfway through your scheduled appointment time may necessitate rescheduling for a later time or another day.
  • Termination Warning: You will receive a termination warning notice upon your third or fourth "No Show." On the fifth "No Show" within a two-year period, we reserve the right to terminate your patient account and refuse service. No-shows older than two years will not be counted towards this termination policy.
